I am a Geriatric Care Manager with knowledge of aging issues. To put it another way, I assist older adults and their families to achieve continuity of care at various stages. For the purpose of reducing the burden on families. Our MCA services not only include Early Intervention but Care Management and Patient Advocacy.

My Mission
I will certainly provide a caring, focused plan for you and your family members. With this in mind, my goal is to help you and your family to not only better understand a diagnosis. But to understand medications, diet, and housing options post-hospitalization or post-rehabilitation. Not to mention, I can facilitate an “aging in place” plan. And, at the same time, assess current services in the home or retirement community.
